Pay a one time $30.00 fee
Receive temporary password and download the medical release of information form
Give the release of information form to your refractive surgeon/eye doctor
You or your surgeon can then input the information into our database with your password; You may also mail it to our office for storage
You or your eye doctor (with your approval) can access your records when needed in the future

We at SAFEGUARD YOUR SIGHT © collect and store this vital information for your eye doctor to use in the future. Your information will be collected managed in strict accordance with HIPAA guidelines for medical privacy and will never be released to any third party unless specified by you. Your personal information will never be sold.

What are doctors saying?

A patient had LASIK 8 years ago and now needs cataract surgery. Optimally, you would like to have the pre-LASIK refraction, topography, and other information to assist with IOL calculations, but the chart at the previous provider's office (or maybe even your own) has been purged or is not available. Would it not be nice if such information were archived online in some format readily accessible by a Web-based interface?
